Complete The Claim Form

Upload Supporting Documents
Attach any documents required to support your claim, such as Images, medical reports, receipts, or invoices. We accept most standard formats (PDF, JPG, PNG, etc.).

Submit Your Claim
Review all information and ensure you’ve included all necessary attachments. Click “Submit” to send your claim for review.
***Once your claim is submitted, you’ll receive a confirmation email. You can check the status of your claim in the web app under the `Claims` section.
